A Waterloo man was arrested on numerous charges after his truck struck a hospital van on John Nolan Drive Monday afternoon, according to Madison police.

The 58-year-old driver of the van owned by Meriter Hospital told police he was turning west onto Olin Avenue from northbound John Nolen Drive when the pickup truck driving south on John Nolen Drive struck the van.

Witnesses told police the truck’s driver was slumped over the steering wheel before fleeing on foot before police arrived.

Officers said they were able to chase the man down and found needles and other drug paraphernalia in his pockets.

Jacob W. Lasiewicz, 27, was arrested on suspicion of fourth-offense operating while under the influence, hit and run, operating while revoked, improper display of license plates, non-registration of vehicle, no auto insurance and running a red light.

Police said Lasiewicz smelled of intoxicants and insisted he had not been in a crash.
