JEFFERSON, Wis. -

Jefferson County sheriff’s officials reported a fatal crash took place on an exit off Highway 16 that left one man dead Friday morning.

The sheriff’s office said the accident occurred about 9:30 a.m. when a medical transport van collided with a car in the Town of Ixonia, causing the van to roll.

The van driver told police the accelerator stuck and he was unable to stop the vehicle while exiting the highway. The van drove through the intersection, over the median and hit the car. The van went into the ditch and rolled.

A car passenger, David Pipkin, 76, of Watertown was flown to Froedtert Hospital in Milwaukee by Flight for Life but died later from injuries suffered in the crash.

A report said three others were involved in the crash: the driver of the car had no injuries. The van’s driver and a passenger had minor injuries. The report does not indicate if they were treated.

Sheriff Paul Milbrath said the crash is still under investigation.
