A Madison hospital has submitted plans to fix problems uncovered after a patient's death.

In a statement to WISC-TV, St. Mary's Hospital said it will continue to work diligently to provide the highest quality patient care in a safe environment.

St. Mary's submitted a copy of its corrective plans to the state Department of Health and Family Services and to a federal agency.

WISC-TV first reported last week that St. Mary's had 10 days to submit plans to fix several deficiencies it was cited for.

The deficiencies were uncovered after the death of a 16-year-old patient led to an investigation.

Jasmine Gant died three weeks ago, on July 5, after she was given the wrong drug during labor.

The plan submitted to the Centers for Medicare and Medicaid Services was just accepted Thursday.

The plan said a team of hospital experts has revised care plans for labor and induction, Caesarean births and high-risk labor.

It also set deadlines for when staff members will be taught the revised plans. The hospital will also revise monitoring of medication, WISC-TV reported.

The "exception report" will now be filled out whenever a scannable medicine is given without being scanned.

The federal report found that a nurse failed to use a bar-code technology to scan Gant's medicine. That scanning indicates whether a patient is getting the right drug.

While federal officials have already approved St. Mary's plan, the state has 10 days to review it.

If approved, the state will conduct a follow-up visit to ensure all issues are resolved, WISC-TV reported.
