Social Security Numbers Printed On Some State Tax Booklets

Thousands of Wisconsinite's Social Security numbers were mistakenly printed on mailing labels for tax forms.

The state said more than 170,000 forms were printed. It believes 115,000 of them actually went through the mail system. The others have been recovered.

The state said the booklets were delivered to people who used the basic Form 1 and filed jointly as married couples for the 2005 tax year.

Some 54,000 booklets were retrieved Wednesday at post offices in Portage, Madison and Oshkosh. The remainder of the booklets are believed to have been delivered.

The Department of Revenue said it provided the confidential information to Ripon Community Printers under a state contract.

This is the third year it has used Ripon Community Printers, and officials said they have never had this problem before.

The printing company said it is trying to determine how the numbers ended up on the labels.

The state said Ripon Community Printers was given the Social Security numbers to make sure the forms go to the correct location but did not say exactly how the information got on the envelopes.

If you receive a booklet with your Social Security number on it, you go to http://www.revenue.wi.gov and click on misprint information.

The state said you should also contact credit bureaus and ask for fraud alerts to be put in place so thieves can't open new accounts in your name.
